No one should fall through the gaps

How can we make the transition from child and adolescent mental health services to adult mental health services more joined-up? This is one of the key questions at the heart of our new project, “No one should fall through the gaps”, which has received funding from Vinnova through the SustainGov programme.

Joined-up care

Each year, young people leave Child and Adolescent Mental Health Services (CAMHS) as they reach adulthood. For many this represents a significant change at a time when their need for support has not gone away. When responsibility for their care moves from one service to another, there is a risk that young people lose contact with mental health services altogether. 

Together with Västra Götalandsregionen, through Sahlgrenska University Hospital, and Region Skåne, through its child and adolescent and adult mental health services, we will explore what could make this transition more seamless and what role Amni could play during this important stage of care. 

Amni is already used across 18 of Sweden’s 21 regions, giving us extensive experience of how mental health services use digital tools for areas such as triage, standardised care pathways and outcome monitoring in everyday clinical practice. This experience, together with the knowledge and data already available through Amni, provides an important foundation for the project. 

We will also make sure that young people themselves have a central role in the project. By involving young people as active co-creators we want to better understand their experiences, needs and perspectives on transitioning between services. 

A first step towards improvement 

The project is a preparatory project running from October 2026 to March 2027. The aim is to develop a robust evidence base for a future implementation project, in which the proposed solution can be tested and implemented in clinical practice across at least two regions. 

At Stretch Care we are both excited and proud to be part of this work. Since the beginning Amni has been developed in close collaboration with healthcare professionals and researchers. We look forward to continuing this journey together and exploring how digital solutions can help create more connected care and make a meaningful difference to patients and healthcare services.
